Coordinating Team: Provides overall guidance and planning for HIA. This team was added in fall of 2023 as a result of feedback received from a recent partnership survey. The team is comprised of representatives from the Greater Bridgeport CHNA funding organizations and HIA task force co-chairs.
HIA Community Coalition: Overall HIA monthly meeting/General HIA meeting. This group meets monthly and is co-chairs by representatives from both Bridgeport Hospital and St. Vincent’s Medical Center, and includes representatives from numerous partner organizations, task force co-chairs and members, and anyone else that lives and works in the Greater Bridgeport that has interest in partnering to improve the health and wellbeing of the region.
Task Forces: Where most of the work of HIA happens. HIA has four active task forces (Access to Care, Behavioral Health, Child Wellbeing, and Healthy Lifestyles) that each meet monthly on a designated date and time. Each task force focuses on a priority area identified through the regional CHNA process and has a 3-year plan, called a CHIP, that guides their work. Task force membership includes representatives from local organizations and communities from throughout the Greater Bridgeport region. Membership for each task force is open to anyone that lives/works in the region and is interested in getting involved.
Subgroups: These groups were created in response to needs identified by two of the task forces.
- Community Health Worker (CHW) Workgroup: Added in 2023, this workgroup was formed as a result of the Child Wellbeing Task Force CHIP and serves as a forum for local CHWs and community ambassadors to network, share resources, and collaborate.
- Community Care Team: This group was seen as a specialized need based on the work of the Behavioral Health Task Force in 2014. The team is a multi-organization group who come together each week, with the sole purpose of identifying people who are high utilizers of Emergency Departments who need significant help in accessing and continuing care.
